ABSTRACT

OBJECTIVE@#To investigate the clinical efficacy of picture archiving and communication system (PACS) and Photoshop assisted isosceles triangle osteotomy and Kirschner wire fixation with tension band in the treatment of cubitus varus in children.@*METHODS@#The clinic data of 20 children with cubitus varus treated with isosceles triangle osteotomy of distal humerus and Kirschner wire fixation with tension band from October 2014 to October 2019, were retrospectively analyzed. There were 13 males and 7 females, aged from 3.2 to 13.5 years old, the median age was 6.65 years old. PACS system was applied for the osteotomy design preoperatively, simulating and measuring the side length of isosceles triangle osteotomy. Then, Photoshop system was used to simulate the preoperative and postoperative osteotomy graphics, which could guide precise osteotomy during operation.@*RESULTS@#All the 20 patients were followed up for 20 to 24 months, with a median of 22.5 months. At the last follow-up, the carrying angle of the affected limb was 5 ° to 13 °, with a median of 8.3 °. The clinical efficacy was evaluated according to the Flynn elbow function score:excellent in 16 cases, good in 2 cases, and fair in 2 cases.@*CONCLUSION@#The treatment of cubitus varus in children by isosceles triangle osteotomy and Kirschner wire fixation with tension band assisted by PACS and Photoshop system has shown good clinical outcome.

Estudo sobre o uso do Picture Archiving and Communication System (PACS) em um Hospital Universitário da Cidade de Belo Horizonte, Minas Gerais- Brasil. 2023. Trabalho de Conclusão de Curso (Mestrado Profissional em Trabalho e Gestão Participativa na Saúde) ­ Escola de Enfermagem da Universidade Federal de Minas Gerais, Belo Horizonte, MG, Brasil. No contexto hospitalar, os exames de imagem são fundamentais para subsidiar e qualificar a tomada de decisão clínica. A radiologia, enquanto especialidade da medicina, tem como objetivo auxiliar os profissionais solicitantes na tomada de decisão para o diagnóstico. A radiologia é essencial porque, em diversas situações, é dela que deriva o tratamento. No contexto do Hospital das Clínicas da Universidade Federal de Minas Gerais (HC-UFMG), utiliza-se o sistema de arquivamento, comunicação de imagens médicas conhecido como- Picture Archiving and Communication System (PACS). Ele é usado no setor da radiologia, permitindo aos outros profissionais solicitantes o acesso e a análise das imagens médicas, de forma eficiente e segura. A pergunta que norteou este estudo foi: Quais as potencialidades e os desafios decorrentes do uso do PACS no setor de Radiologia do Hospital de Clínicas da Universidade Federal de Minas Gerais (HC-UFMG)? O objetivo da pesquisa foi analisar como foi a implantação e a implementação do uso do PACS no setor de radiologia do HC-UFMG. Trata-se de um estudo de natureza qualitativa, exploratória e descritiva. O cenário da pesquisa foi o setor de Radiologia do HC-UFMG. Os participantes foram nove médicos radiologistas do referido setor. Foi adotado como critério de inclusão: médicos radiologistas efetivos (médicos concursados e que não estavam em período de férias e ou afastados) que utilizavam o sistema PACS no setor da Radiologia do HC-UFMG. Como critérios de exclusão adotou-se: médicos que não compunham o quadro efetivo no setor, ou seja, que eram voluntários ou residentes; foram excluídos também dois médicos que estavam de férias e dois que estavam de licença formal durante o período da coleta de dados. A coleta de dados foi realizada por meio de entrevistas com roteiro semiestruturado e levantamento documental. Realizou-se a análise dos dados das entrevistas pela técnica de Análise de Conteúdo e, dos documentos, por análise documental. A pesquisa foi submetida ao Comitê de Ética em Pesquisa da Universidade Federal de Minas Gerais (COEP/UFMG) e aprovada pelo CAAE 67471123.0.0000.5149, após aprovação pelo Departamento de Gestão em Saúde e anuência da Gerência de Ensino e Pesquisa (GEP) do HC/UFMG. Os principais resultados indicam que o PACS é considerado uma ferramenta central e eficiente pelos radiologistas entrevistados, por facilitar o acesso e a visualização dos exames de imagem, agilizar a tomada de decisão clínica e contribuir para uma assistência mais coordenada e eficaz. Conclui-se que a relevância deste estudo se reflete na contribuição para a compreensão das potencialidades e desafios relacionados ao uso do PACS em um hospital universitário, fornecendo subsídios para aprimorar a gestão em saúde e a assistência aos pacientes. A study on the use of the Picture Archiving and Communication System (PACS) at the Hospital the Federal University of Belo Horizonte city of Minas Gerais (HC-UFMG). 2023. Dissertation (Professional Master's in Work and Participatory Management in Health) - School of Nursing, Federal University of Minas Gerais, Belo Horizonte, MG, Brazil. In the hospital context, imaging tests are essential to support and qualify clinical decision- making. Radiology, as a specialty of medicine, aims to assist referring physicians in making diagnostic decisions. Radiology is essential because in several situations, treatment is derived from it. In the context of Hospital das Clínicas of the Federal University of Minas Gerais (HC- UFMG), we used the Picture Archiving and Communication System (PACS) for archiving and communication medical images. Which is used in the radiology sector and allows other requesting professionals to access and analyze medical images efficiently and securely. A question that guided this study was: What are the potentialities and challenges resulting from the use of the future Archiving and Communication System (PACS) in the Radiology sector Clinical Hospital of Federal University of Minas Gerais (HC-UFMG)? The purpose of the research was to analyze the potentialities and challenges resulting from the use of PACS in the radiology at HC-UFMG. This is qualitative, exploratory and descriptive. The research scenario was the Radiology sector of HC-UFMG. The participants were nine radiologists from that sector. The following inclusion criteria were adopted: effective radiologists (doctors who were on public service and who were not on vacation and/or away) who used the PACS system in the Radiology sector of HC-UFMG. As exclusion criteria it was adopted: physicians who did not make up the effective physicians in the sector, that is, who were volunteers or residents; were excluded also two doctors who were on vacation and two who were on formal leave during the period of data collection. Data collection was carried out through interviews with a script semi-structured and documentary survey. Data analysis of the interviews was carried out by the Content Analysis technique and documents by document analysis. The research was submitted to the Research Ethics Committee of the Federal University of Minas Gerais (COEP/UFMG) and approved by CAAE 67471123.0.0000.5149, after approval by Department of Health Management and approval of the Teaching and Research Management (GEP) of the HC-UFMG The main results indicate that the PACS is considered a tool central and efficient by the radiologist interviewed, as it facilitates access and visualization of imaging exams, streamline clinical decision-making and contribute to more coordinated and effective. It is concluded that the relevance of this study is reflected in the contribution to the understanding of the potentialities and challenges related to the use of PACS in a hospital university, providing subsidies to improve health management and assistance to patients. The technical product of this study is a technical report, to be presented to the management from HC-UFMG. Keywords: Picture Archiving and Communication System (PACS). Technology. Efficiency. ABSTRACT

Objective:To evaluate the effect of PACS system (picture archiving and communication system) in the standardized residency training of cardiovascular medicine.Methods:Fifty-nine residents taking the standardized residency training of cardiovascular medicine in Changhai Hospital from 2018 to 2019 were randomly divided into PACS teaching group ( n = 30) and traditional teaching group ( n = 29). The research group adopted the PACS system for the teaching of cardiovascular medicine, and the control group took the traditional teaching method. The teaching effect was evaluated by theoretical examination, imaging examination and questionnaire. SPSS 19.0 was used for t test and chi-square test. Results:The theoretical and film reading scores of the PACS teaching group were significantly higher than those of the control group [(87.70 ± 6.52) vs. (80.55 ± 8.63); (86.67 ± 6.33) vs. (77.48 ± 10.29), P < 0.05)]. The results of the questionnaire showed that PACS teaching method was helpful for residents to master cardiovascular knowledge, arouse their learning interest and improve their clinical thinking ability. The satisfaction with the teaching method in the PACS teaching group was significantly higher than that in the traditional teaching group [(9.22 ± 0.44) vs. (8.26 ± 0.72), P < 0.05]. Conclusion:The application of PACS system in the teaching of cardiovascular medicine can significantly improve the teaching effect of residents.

ABSTRACT

El Sistema para el Almacenamiento, Transmisión y Visualización de Imágenes Médicas XAVIA PACS, desarrollado por la Universidad de las Ciencias Informáticas, se utiliza en múltiples instituciones de salud de Cuba. El propósito de estas herramientas es manejar la imagen médica, por lo que no gestionan la información que se capta en los servicios de diagnóstico por imágenes, entre los que se encuentran los datos del paciente, del estudio imagenológico y el informe diagnóstico. En este trabajo se exponen los resultados del desarrollo de un sistema informático para la gestión de la información imagenológica integrado al sistema XAVIA PACS para garantizar el control automatizado de la información en los servicios de diagnóstico por imágenes. Para el desarrollo previsto fue determinante la necesidad de reutilizar el código fuente ya implementado en el sistema XAVIA PACS; se seleccionó Microsoft.NET Framework, ASP.NET 3.5, C# 3.5 como lenguaje de programación, PostgreSQL como sistema de gestión de Base de Datos. Adicionalmente, fueron tenidos en cuenta la portabilidad, el procesamiento complejo interno, la facilidad de la instalación, y la facilidad del cambio. El sistema desarrollado automatiza la gestión de las listas de trabajo en los servicios de diagnósticos por imágenes, la gestión de la información del paciente, las citaciones, la planificación del trabajo de las modalidades médicas, y especialistas de radiología. La implantación del sistema desarrollado en diferentes instituciones de salud que emplean el sistema XAVIA PACS, validó la pertinencia y aplicabilidad del mismo(AU)


The XAVIA PACS Medical Images Storage, Transmission and Visualization System, developed by the University of Computer Sciences is used in multiple Cuban health institutions. The purpose of these tools is to manage the medical image, so they do not manage the information that is captured in the diagnostic imaging services, among which are the patient's data, the imaging study and the diagnostic report. This article presents a computer system for the management of imaging information integrated into the XAVIA PACS system to ensure automated control of information in diagnostic imaging services. For the planned development, the need to reuse the source code already implemented in the XAVIA PACS system was decisive; Microsoft.NET Framework, ASP.NET 3.5, C # 3.5 were selected as the programming language, PostgreSQL as the Database management system. Additionally, portability, complex internal processing, ease of installation, and ease of change were considered. The developed system automates the management of work lists in diagnostic imaging services, the management of patient information, appointments, work planning of medical modalities, and radiology specialists. The implementation of the system developed in different health institutions that use the XAVIA PACS system, validated its relevance and applicability(AU)

ABSTRACT

Una de las campañas más reconocidas en el mundo es la lucha contra el cáncer, siendo el sistema renal uno de los más afectados por esta patología. El carcinoma de células renales (CCR), el más común de cáncer renal en los adultos, representa la sexta causa de muerte por cáncer. Debido al aumento en el uso de las técnicas de diagnóstico por imagen, las lesiones renales pueden ser diagnosticadas en forma incidental aproximadamente en 50% de los casos. Cuba apuesta por el uso de la tecnología en la salud y en la Universidad de las Ciencias Informáticas (UCI) se ha desarrollado un sistema para el almacenamiento, transmisión y visualización de imágenes médicas (XAVIA PACS), el cual se encuentra implantado en varios hospitales del país, pero no cuenta con alternativas para realizar la detección del CCR en imágenes tomográficas, haciendo más lento el diagnóstico, lo que se traduce en menos posibilidades para el paciente. La presente investigación tiene como objetivo realizar un análisis sobre las principales técnicas de segmentación y procesamiento para la detección de carcinomas renales en imágenes de tomografías abdominal, que propicie a los equipos de desarrollo contar con la base teórica necesaria para enfrentar el problema en cuestión. Para ello se realizó un análisis documental sobre trabajos relacionados con la temática y que propician soluciones al problema. Se estudiaron algoritmos y técnicas computacionales efectivas para la segmentación y procesamiento de imágenes abdominales. Como resultado de la investigación se obtuvieron los algoritmos más acordes para el sistema XAVIA PACS y el contexto médico cubano(AU)


One of the most recognized campaigns in the world is the fight against cancer, the kidney system being one of the most affected by this pathology. Renal cell carcinoma (RCC), the most common form of kidney cancer in adults, represents the sixth leading cause of cancer death. Due to the increased use of diagnostic imaging techniques, kidney injuries can be diagnosed incidentally in approximately 50% of cases. Cuba is committed to the use of technology in health and a system for the storage, transmission and display of medical images (XAVIA PACS) has been developed at the University of Computer Sciences (UCI), which is implanted in several hospitals of the country, but it does not have alternatives to detect RCC in tomographic images, slowing down the diagnosis, which translates into fewer possibilities for the patient. The objective of this research is to carry out an analysis on the main segmentation and processing techniques for the detection of renal carcinomas in abdominal tomography images, which provides development teams with the theoretical basis necessary to face the problem in question. For this, a documentary analysis was carried out on works related to the subject and that provide solutions to the problem. Algorithms and effective computational techniques for the segmentation and processing of abdominal images were studied. As a result of the research, the most suitable algorithms for the XAVIA PACS system and the Cuban medical context were obtained(AU)

BACKGROUND: Herniated cervical intervertebral disc volume measurement is an important parameter for quantitative evaluation of cervical disc degeneration, but it faces a lot of problems such as different measurement standards and the undefined measurement error range. OBJECTIVE: To investigate the accuracy of PACS software in measuring cervical disc volume, provide reliable measurement methods and accurate data support for clinical observation and research on cervical disc volume change and degeneration. METHODS: The error rate was obtained by repeated measurements of the normal saline with a known volume of 5.0 mL by means of PACS software. With reference to this error rate, volume changes of cervical disc herniation before and after cervical microendoscopic laminoplasty were “monitored” and analyzed in 30 cases. This study was approved by the Ethics Committee of the First Affiliated Hospital of Zhengzhou University (approval No. 2019-KY-274) on September 26, 2019. RESULTS AND CONCLUSION: (1) For the measurement of normal saline with known volume, it was found that the error rate of measurement by PACS software was ±5%, suggesting that the measurement of cervical disc volume by PACS software is a simple and accurate method. (2) After cervical microendoscopic laminoplasty, there were 70 patients with reduced cervical disc volume reduction absorption rate of 5%-100%, and the absorption ratio was 76.1% (70/92). The volume increased by 11, but the increase was not more than 5% in the patients with cervical disc herniation after treatment. (3) The spontaneous disappearance or reduction of the herniated cervical disc after cervical microendoscopic laminoplasty was as early as 7 days, and the longest was 76 months. (4) The effects were excellent in 11 cases, good in 15 cases, and fair in 4 cases. The excellent and good rate was 86.7%.

ABSTRACT

OBJECTIVE@#To explore the integration method and technical realization of artificial intelligence bone age assessment system with the hospital RIS-PACS network and workflow.@*METHODS@#Two sets of artificial intelligence based on bone age assessment systems (CHBoneAI 1.0/2.0) were developed. The intelligent system was further integrated with RIS-PACS based on the http protocol in Python flask web framework.@*RESULTS@#The two sets of systems were successfully integrated into the local network and RIS-PACS in hospital. The deployment has been smoothly running for nearly 3 years. Within the current network setting, it takes less than 3 s to complete bone age assessment for a single patient.@*CONCLUSIONS@#The artificial intelligence based bone age assessment system has been deployed in clinical RIS-PACS platform and the "running in parallel", which is marking a success of Stage-I and paving the way to Stage-II where the intelligent systems can evolve to become more powerful in particular of the system self-evolution and the "running alternatively".

ABSTRACT

BACKGROUND: Osteomyelitis is an intraosseous inflammatory disease characterized by progressive inflammatory osteoclasia and ossification. The use of quantitative analysis to assist interpretation of osteomyelitis is increasingly being considered. The objective of this study was to perform early diagnosis of osteomyelitis on digital panoramic radiographs using basic functions provided by picture archiving and communication system (PACS), a program used to show radiographic images. METHODS: This study targeted a total of 95 patients whose symptoms were confirmed as osteomyelitis under clinical, radiologic, pathological diagnosis over 11 years from 2008 to 2017. Five categorized patients were osteoradionecrosis, bisphosphonate-related osteonecrosis of jaw (BRONJ, suppurative and sclerosing type), and bacterial osteomyelitis (suppurative and sclerosing type), and the control group was 117 randomly sampled. The photographic density in a certain area of the digital panoramic radiograph was determined and compared using the “measure area rectangle,” one of the basic PACS functions in INFINITT PACS® (INFINITT Healthcare, Seoul, South Korea). A conditional inference tree, one type of decision making tree, was generated with the program R for statistical analysis with SPSS®. RESULTS: In the conditional inference tree generated from the obtained data, cases where the difference in average value exceeded 54.49 and the difference in minimum value was less than 54.49 and greater than 12.81 and the difference in minimum value exceeded 39 were considered suspicious of osteomyelitis. From these results, the disease could be correctly classified with a probability of 88.1%. There was no difference in photographic density value of BRONJ and bacterial osteomyelitis; therefore, it was not possible to classify BRONJ and bacterial osteomyelitis by quantitative analysis of panoramic radiographs based on existing research. CONCLUSIONS: This study demonstrates that it is feasible to measure photographic density using a basic function in PACS and apply the data to assist in the diagnosis of osteomyelitis. Objective To research an ultrasound imaging workstation based on DICOM so as to construct a patient-centered effective digital hospital.Methods Several interfaces of ultrasound imaging workstation based on DICOM were showed and their advantages and disadvantages were analyzed.Ultrasound imaging workstation based on DICOM was also compared with that based on video capture card.Results DICOM-based ultrasound imaging workstation gained advantages over the one based on video capture card in availability,resolution and management.Conclusion The workstation may contribute to enhancing clinical efficiency,promoting digital hospital and regional medical image sharing,and thus is worthy popularizing practically.

Objective To develop a PACS covering the whole hospital.Methods Communication between PACS and the existing HIS was realized with Web technology,SOA,Oracle database and barcode technology.esults The electronic information from HIS contributed to real-time,automatic and one-to-one charging by PACS,and the examining results and images were returned to HIS to implement the integration of data set,application set and flow set,and statistical analysis was carried out on the examining results,then a set of PACS/RIS with complete functions came into being.Conclusion PACS/RIS simplifies the process for visiting a doctor,decreases waiting time,transmits data on the detection equipment in time and implements paperless image report.

Objective To build an advanced clinical case collection and teaching system for the department of radiology, and thus to facilitate the scientific research and teaching.Methods By combining several free software, we built a network version of the case collection system with simple web pages, to achieve centralized storage of case text and image data, thus to facilitate the research and teaching work.Results This case collection system quickly saved text data and image data through simple operation, supported parallel work without the need for complex configuration on the client, and had good support for wide area network.This system had stable system operation and could be customized to meet the clinical needs well.Conclusion Using free software to establish the network version of the case collection and teaching system is low cost and has no legal risk.This system is a simple but powerful tool, and can meet the needs of hospitals with different sizes, which is a more perfect case collection system.

Objective To develop a piece of software to execute batch data input for PACS.Methods The running modes of HIS and PACS and related databases were analyzed.The oracle call interface (OCI) of hypertext preprocessor (PHP) was used to expand the access database,so that multi pieces of data were input into PACS database in one time.Results The batch input was realized for PACS,and the time consumed was decreased greatly while the accuracy and utilization rate of the data were guaranteed in the catheterization room.Conclusion The software expand the function of PACS with low cost and high efficiency,and meets the requirements of the catheterization room and other departments.

ABSTRACT

Picture archiving and communication system (PACS) was used to perform archiving and retrospective cases report presentation in resident radiologist standardized training, and the current conven-tional article reviewing case report presentation form was compared. The results showed the scores of quiz and questionnaire of using archiving and retrospective cases report based on PACS were higher than using the current conventional form (P<0.05). This type of case report presentation can improve overall capability of resident radiologist standardized training including imaging diagnose,research and educational skill.

Objective To analyze the application status and problems of PACS so as to facilitate its future development.Methods The application of PACS in some hospital from June 2015 to May 2016was reviewed,and considerations were taken on the hospital conditions to explore the application status and problems of PACS in China.Results There were a number of problems found in Department of International Diagnosis and Treatment,including 13-a active time of image acquisition devices,6.8% of the workstations and 14.2% of the image acquisition devices having no interface with PACS,and 2.9% of the images non-uploaded to PACS.Conclusion PACS in China has to emphasize on stability,high efficiency,safety and reliability to facilitate its future application to medicine,scientific research and teaching.

ABSTRACT

Objective To analyze the application status and problems of PACS so as to facilitate its future development.Methods The application of PACS in some hospital from June 2015 to May 2016was reviewed,and considerations were taken on the hospital conditions to explore the application status and problems of PACS in China.Results There were a number of problems found in Department of International Diagnosis and Treatment,including 13-a active time of image acquisition devices,6.8% of the workstations and 14.2% of the image acquisition devices having no interface with PACS,and 2.9% of the images non-uploaded to PACS.Conclusion PACS in China has to emphasize on stability,high efficiency,safety and reliability to facilitate its future application to medicine,scientific research and teaching.

ABSTRACT

En nuestro país se lucha por la soberanía tecnológica y se impulsa la utilización del software libre. Por esto y con un mejor diseño de las soluciones implementadas por el Centro de Biofísica Médica, el imagis 2.0, como paquete de soluciones de un sistema de almacenamiento y transmisión de imágenes médicas, fue desarrollado bajo la plataforma Linux, haciendo uso de herramientas libres, convirtiéndolo en un sistema más eficiente, estable y robusto. Con este trabajo pretendemos primero realizar un breve recorrido por la historia de los sistemas de almacenamiento y transmisión de imágenes médicas en nuestro país, para luego adentrarnos en una solución implementada bajo plataforma libre que sirve de base a la implementación para especializaciones médicas, exponiendo su uso en los procesos radioterapéuticos(AU)


In our country we fight for technological sovereignty and the use of free software is promoted. For this and with better design solutions implemented by the Center of Medical Biophysics, the imagis 2.0, as a package of solutions of storage and transmission of medical images systems, was developed under the Linux platform, using free tools, making it more efficient, stable and robust. In this paper we first present a brief review of storage and transmission of medical images history in our country, for then get into a deployed under open platform solution that can be using as framework for the implementation for medical specializations, exposing it use in radiotherapeutic procedures(AU)

ABSTRACT

Abstract. RIS PACS systems have become indispensable for managing, distributing and archiving digital images. This study evaluated the impact of Computed Tomography in the Clínica Alemana of Santiago's PACS image service. The PACS database was reviewed using IMPAX BI software, and 2,267,683 studies between 2005 and 2014 were examined. Computed Tomography examinations made up 10.67% of the studies archived in PACS, but its weight in TB is 50.10% of the total. The impact of the studies of Computed Tomography were not properly dimensioned or anticipated, which has impacted on the expected availability for one year online studies. This study has adequately assessed the impact of Computed Tomography in PACS’ storage availability, and how it should be considered when planning the implementation of a PACS.


Resumen. Los sistemas RIS PACS se han vuelto indispensables para el manejo de las imágenes digitales, su distribución y archivo. Esta investigación evaluó el impacto de la tomografía computarizada en el PACS del servicio de imágenes de la Clínica Alemana de Santiago. Se revisó la base de datos del PACS a través de la herramienta de software IMPAX BI, de los 2.267.683 estudios del período 2005-2014. Los exámenes de tomografía computarizada representan el 10,67% de los estudios archivados en el PACS, pero su peso en TB es el 50,10% del total. El impacto de los estudios de tomografía computarizada no se ha dimensionado ni previsto adecuadamente, lo que ha impactado en la disponibilidad esperada para los estudios de un año on line. Este estudio ha permitido valorar adecuadamente el impacto de la tomografía computarizada en la disponibilidad del PACS, y cómo debe ser considerado al planificar la implementación de un PACS.

ABSTRACT

Architectural changes with Tianyi cloud storage of China Telecom were made to the up/ down loading of the image-text mega data created in the picture archiving and communication systems (PACS)of the hospital.Thanks to the changes, the data volume uploaded by the PACS of the hospital reached 160G per day, with average upload speed up to 5.3M/second;supporting online query of all image-text data of patients.The authors hold cloud storage technology as safe and reliable, and such other merits as supporting data encryption, automatic offsite redundancy, almost online real-time read/write access,almost infinite extendibility, fast self-healing and off-site disaster-tolerant backup.
